Contextualised care: Are we all trying to be Waitrose?

We can't all be Waitrose, yet increasingly this is what small animal practices aspire to be to the exclusion of everything else. Have we lost something in this constant drive to only serve those with the means to purchase human-quality healthcare for their pets? It is clearly impossible to be all things to all people, so why have we abandoned pragmatism and a variety of practices with different service offerings? Where are the budget practices? Who is going to work in small local community practices with a service tailored for the specific needs of that community?
